Please read the instructions below before filling out the form:
1. Enter Full Name.
2. Enter a valid Email Address on which you want to receive account statement.
3. Enter Account Number in the format. eg. 0XXXXX01
4. Enter Mobile Number which is registered with EDC Limited (it should be 10 digits only).
5. Enter  Account Statement From Date. (Account statements can be generated from 01-Apr-2014 onwards)
6. Enter Account Statement To Date. (To Date cannot be more than the Current Date)
